Nokia Launches its Patient Care Dashboard to Scale Up Remote Patient Monitoring

Patients today are more engaged than ever in their own health care. Unfortunately for providers, that care increasingly takes place outside of the doctor’s office. When patients leave the exam room, their biometric data leaves with them and their doctor has no way of knowing how that patient is doing in between visits. That’s why Nokia is proud to announce a new Patient Care Dashboard that connects patients with their doctors at home and on the go.

With this new dashboard, care teams can view real-time patient-generated data on both an individual and population level. Based on patient readings from easy-to-use Withings health monitoring devices, care teams can see who needs an intervention and who just needs a high-five. They can view trends, and more importantly, find opportunities to improve care and reduce costs. Nokia Patient Care enables effective monitoring and managing of health outcomes for an entire patient population.
Nokia health monitoring devices (currently sold under the name Withings) are already being leveraged by major care providers in the US including Ochsner Health System, Stanford Health Care, University of Pennsylvania, Weill Cornell Medical Center and Mayo Clinic, who are integrating the patient-generated data within their Electronic Health Records. Now, care teams at the Fairview Health System and the University of Kentucky will layer in this new Patient Care Dashboard to engage patients right when they need help. The platform is also being used by the National Health Service in the UK to better understand hypertension and the role of remote monitoring in lowering hypertension rates with the NHS care teams’ support.

Connecting care teams to patients at home

For the first time, care teams can now access patient-generated data along a comprehensive set of health metrics on a secure and easy to interpret dashboard. Metrics include key vital signs such as weight, blood pressure, heart rate and even pulse wave velocity in addition to traditional health metrics such as activity and sleep. The care team sets custom flags to take action quickly and avoid unnecessary hospitalizations.
For example, a care team can choose to see when patients with congestive heart failure gain more than three pounds in one day, or five pounds in three days. Such weight gain is typically an early sign of cardiac decompensation and requires a provider-led intervention. With Nokia’s new Patient Care Dashboard, care teams would be able to adjust the patient’s care plan and outreach accordingly.

Managing chronic diseases remotely is cost-efficient

Beyond acute events, Nokia’s Patient Care Dashboard enables care teams to better manage patients with chronic conditions in the long term. Approximately one in three Americans suffers from hypertension but only half effectively control their blood pressure. And uncontrolled blood pressure increases the risk of heart disease such as congestive heart failure (CHF) by up to 50 percent. To truly change the tide of hypertension and other chronic conditions, care teams require constant feedback on a patient’s health status and gain visibility into critical values.

This dashboard combines Nokia’s award-winning consumer health devices and world-class analytics to provide stratified patient information without creating alert and data fatigue.
